What exactly determines that? Is that if I invoke the script with Siri or is there some setting in the app or script? This is in the sample comic script for xkcd, but it is not reading the alt text aloud.

The example script reads it aloud when I ask Siri to run the script.
The docs are succinct, but accurate.
Aha, I looked at the docs, but didn’t pull down to see the search and didn’t see it under speech. Doh.
